Diclofenac Potassium (50 mg), Serratiopeptidase (10 mg)

CYS-D 50mg / 10mg Tablet is a combination of two medicines. It is used to reduce pain and inflammation in conditions such as rheumatoid arthritis, ankylosing spondylitis, and osteoarthritis. It is also used to relieve muscle pain, back pain, toothache, ear and throat pain.

Serratiopeptidase is a proteolytic enzyme used to reduce inflammation, has antiademic, analgesic, fibrinolytic, and caseinolytic properties. It is used to treat muscle aches, back pain, toothache, menstrual cramps, and sports injuries. It also reduces pain, swelling, and stiffness of joints due to arthritis. Reducing these symptoms helps you more in your normal daily tasks. This drug is known as an non-steroidal anti-inflammatory drug (NSAID).

Mechanism of Action

Diclofenac Potassium, safe and potent NSAID is used for pain relief in heart and diabetes patients. When combined with a proteolytic enzyme serratiopeptidase, pain, inflammation, and tenderness rapidly resolve.

The primary mechanism responsible for its anti-inflammatory/antipyretic/analgesic action is inhibition of prostaglandin synthesis by inhibition of cyclooxygenase (COX).

Serratiopeptidase 

It is a proteolytic enzyme derived from Serratia spp. Serratiopeptidase is a proven proteolytic enzyme that has direct mucolytic action, breaks down mucus and promotes expulsion, and helps in antibiotic penetration.

Pharmacodynamics 

High enzymatic activity and potent anti-inflammatory effects with inhibition of prostaglandin and bradykinin hydrolysis of COX-2. It also helps in the rapid healing of trauma.

Indications

Pain and edema, Tendonitis, Dental extractions, Episiotomy, Breast engorgement, tonsillectomy, Oedema after ocular surgery, Bronchiectasis.

Half-life

1.2-2 hr (35% of the drug enters enterohepatic recirculation)

Onset of action

1-hour duration of action – 12 hours.

Pharmacokinetic Properties:

Bioavailability – 100%; Protein – more than 99% binding; Metabolism – liver, no active metabolites exist; Half-life – 1.2–2 hr (35% of the drug enters enterohepatic recurrence); Excretion – bile, only 1% in urine.

Precautions:

In patients with renal failure or liver failure. when taking any anti-inflammatory agent on an empty stomach, the physician should alert the patient to abdominal pain. Such enzyme preparations should be used in patients with gastritis and a history of gastric ulcers, as well as in patients taking blood thinners.

Dosage:

Adults-5 to 10mg tab 3 times daily. 10- 30 mg tab /day.

8 to 10 hrs

Adverse Reaction:

Diclofenac is one of the better-tolerated NSAIDs. 20% of patients on long-term treatment suffered with side effects, out of which  only 2% had to discontinue the drug, mostly due to gastrointestinal complaints. Contraceptive hypersensitivity against a history of diclofenac, bronchospasm, shock, rhinitis, urticaria) after the use of aspirin or another NSAID, third-trimester pregnancy.

Blood coagulation abnormalities disturbance or under treatment with anticoagulants.
